Job Description

Riggs Community Health Center is seeking highly trained, independent Family or Adult Nurse Practitioner with a background in chronic disease management who enjoys teaching and practicing primary care with a vulnerable population. Riggs Community Health Center’s service area population in Tippecanoe County is low-income – amounting to nearly 70,000 low-income individuals of the approximately 192,000 residents. Of these low-income individuals, only 18 percent are being served by community health centers. Tippecanoe County is in west-central Indiana, a mix of urban and rural areas with metropolitan cities, academic communities, rich farmlands and a meandering river. Tippecanoe County is designated as a Medically Under-served Area/Population (MUA/P), as well as a single county low-income and mental health Health Professional Shortage Area (HPSA), with a primary care HPSA score of 15, dental HPSA score of 21, and a mental health HPSA score of 20.

Lafayette Area

Salary: $120,000/year
No Call

Monday – Friday, some Saturdays (8a – 12p) on rotation

Job Requirements:

· 3+ years experience in related field
. Masters in Nursing required
. Graduate of an accredited school of nursing and nurse practitioner program
· Current Indiana registered nurse license
· Certified as a nurse practitioner by an approved certifying organization
· Indiana CSR and DEA certification preferred
· CPR certification
· Excellent interpersonal skills
· Bilingual in Spanish helpful

Essential job duties include:

Clinical Care:

Utilizes appropriate and effective patient care plans
Manages patients to assure continuity of care
Provides whole person care, which includes referrals to other health care providers
Maintains adequate, concise and legible documentation of patient care
Provides and documents periodic health examinations
Provides appropriate patient education
Demonstrates appropriate work habits which includes: arriving on time, returning phone calls same day, utilizing appropriate infection control measures, documentation in a timely manner, maintaining patient confidentiality, and making scheduling changes only through the Medical Director

Team Contributions:

Works with others to develop effective solutions to issues
Provides support and training to co-workers when needed
Models and encourages positive staff interactions
Participate in assigned QA
Regularly attends Team/Clinical meetings

Administrative Activities:

Participates in nurse practitioner performance audits as assigned
Assists in the development and maintenance of Riggs CHC practice guidelines (protocols) as assigned
Participates in the Quality Assurance Committee as assigned
Attends required in-service meetings
Attends other meetings as assigned by Medical Director

Community Oriented Activities:

Makes presentations in the community when requested
Attends annual board, staff, and volunteer meetings
Represents the provider staff at the Riggs CHC Board or other local organizations if requested

Professional Networking Activities:

Teaches students as assigned
Attends yearly continuing education meeting
Attends regional or national Community Health Center meetings as requested

Personal Work Relationships:

Presents self in professional manner at all times through speech, conduct, and dress
Demonstrates initiative, enthusiasm, and organizational skills
Follows Riggs CHC policies, procedures and protocols
Maintains strict Riggs CHC confidentiality

Physical Demands and Work Environment:

Clinic may be crowded and noisy at times
Flexibility and adaptability to change is essential
Exposure to communicable illnesses/diseases
Exposure to hazardous chemical and bio-hazardous materials
Handling of sharp instruments and needles
Some emotional stress resulting from the diversity and intensity of patients and staff
Some vulnerability to patient-induced injury
Requires the ability to distinguish letters, numbers and symbols
Lifting of 20 pounds regularly and up to 50 pounds infrequently
Risk Level I
